Clinical Care Coordinator

Job OverviewThis role is part of our Integrated Care Team, working closely with GP Practices to manage a defined community caseload via Nursing, mental health, Physiotherapy, or Occupational Therapy interventions.
Main Responsibilities- Manage a community caseload for a group of GP Practices, providing high-quality care and promoting timely discharge into the community.
- Work collaboratively with the Integrated Care Team to prevent unnecessary admissions to acute hospitals and support re-enablement, rehabilitation, health protection, and promotion programmes.
- Develop and implement patient-centred care plans, prioritising their needs and preferences.
