**Job Title:** Nurse Care Coordinator

**Location:** Ringwood, UK

**Salary:** Up to £45,864 per annum (approximately £19.00 per hour)

We are seeking an experienced Registered General Nurse (RGN) or Registered Mental Health Nurse (RMN) to join our team at Appoint Healthcare, a leading national provider of health and social care services for young people and adults with mental health needs.

About the Role:

As a Nurse Care Coordinator, you will play a critical role in ensuring seamless delivery of care to patients with complex mental health needs. You will be responsible for assessing patients' physical and mental health needs, developing individualized care plans, and coordinating care with other healthcare professionals.

Responsibilities:

  • Conduct comprehensive assessments of patients' physical and mental health needs
  • Develop and implement individualized care plans that address patients' physical and emotional needs
  • Coordinate care with other healthcare professionals to ensure seamless delivery of care
  • Monitor patients' progress and adjust care plans as needed
  • Document all aspects of patient care accurately and timely

Requirements:

  • RGN or RMN qualification and valid NMC registration
  • A minimum of 6 months' experience as a nurse in the UK
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Commitment to delivering high-quality patient care
