About the Role

We are seeking a highly motivated and experienced Care Coordinator to join our team at Essex Partnership University NHS Foundation Trust. As a Care Coordinator, you will play a vital role in providing high-quality care and support to individuals experiencing a first episode of psychosis.

Key Responsibilities
  • Undertake care coordination of a caseload of clients, ensuring that their needs are met and their care plans are implemented effectively.
  • Conduct initial health and social needs assessments, and participate in multi-disciplinary team meetings to allocate clients to care plans.
  • Develop and implement recovery-based care plans, working collaboratively with clients, carers, and other healthcare professionals.
  • Prioritize and manage workload to ensure effective management of a defined group of clients with complex needs.
  • Participate in annual audits and contribute to the improvement of services provided to clients.
Requirements
  • Appropriate professional registration (e.g. RMN, RNLD, or equivalent)
  • Experience of working with individuals experiencing a first episode of psychosis
  • Understanding of First Episode Psychosis/wait time standards
  • Excellent communication and interpersonal skills
