Job Opportunity: Community Mental Health Practitioner

We are seeking a highly skilled Community Mental Health Practitioner to join our Perinatal Community Mental Health Team. As a vital member of the team, you will play a critical role in enhancing mental health outcomes for individuals and families through the application of social work interventions and representation of the social work perspective.

This is a unique opportunity to make a positive impact on the lives of vulnerable individuals and contribute to the development of the social work workforce within the trust. If you are dedicated to delivering high-quality care and passionate about social work, we invite you to apply.

The successful candidate will hold a professional qualification in Social Work, current registration with Social Work England, and a willingness to pursue ongoing training and education that enhances service delivery. A full UK driving licence and vehicle for business use are also required for this post, with reasonable adjustments made for disabled individuals.

Main Responsibilities:
  1. Conduct comprehensive assessments and provide access to social work interventions for patients and their families experiencing significant mental health challenges.
  2. Deliver tailored, time-sensitive, and intensive social work interventions within a community setting.
  3. Demonstrate exceptional clinical and managerial leadership skills within the team, offering education and supervision to junior staff members and other professionals.
  4. Foster relationships and collaborate with statutory and third-sector organisations to facilitate family recovery.
Requirements:

The ideal candidate will possess outstanding communication and interpersonal skills, enabling them to thrive in a multidisciplinary team environment. They must also demonstrate a deep understanding of current legislation and clinical risk assessments, as well as emotional intelligence and conflict resolution skills.

An elevated level of emotional intelligence and proficiency in assessment, intervention, and evaluation of patient care are essential. The ability to adapt and manage conflict issues is desirable, as is awareness of national and local factors influencing care.

Remuneration:

£32,000 - £42,000 per annum
