We are seeking a motivated and enthusiastic practitioner to join our Northern Learning Disabilities Team in North Norfolk. Our service provides an integrated Health and Social Care approach to assessment and support in the community.



About You




  • You will be able to work under pressure, be well-organised and effectively prioritise.
  • Excellent communication skills, record keeping and keyboard skills are essential.
  • A commitment to excellent customer service and multi-agency working is key.
  • You will build strong partnerships with service users and their families.


About Us



Norfolk County Council and Norfolk Community Health and Care Trust deliver a single service for adults with a Learning Disability. Our integrated Health and Social Care team provides specialist Learning Disability support to adults in the community.
