About the Role

We are seeking a highly motivated and experienced Pathway Advisor to join our Leaving Care Team at Swindon Borough Council. As a Pathway Advisor, you will play a vital role in supporting young people aged 16 to 25 who have been in our care, helping them to live independent lives and achieve their full potential.

Key Responsibilities

  • Develop and implement individualized Pathway Plans to address the changing needs of young people
  • Provide guidance and support to young people, including advice on education, employment, and life skills
  • Collaborate with other agencies and services to ensure a seamless transition for young people
  • Monitor and evaluate the effectiveness of Pathway Plans and make adjustments as needed

Requirements

  • Fluency in the English language (as a requirement of Part 7 of the Immigration Act)
  • Experience working with young people who have been in care
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
