Support Dementia Patients as a Post Diagnostic Link Worker

Are you passionate about providing high-quality support to individuals living with dementia and their families?

In this role, you will be part of the NHS Scotland's commitment to enabling people to live well with dementia.

You will work within the Older Adult Community Mental Health Team, East Lothian Community Hospital, supporting patients in line with Alzheimer Scotland's 5-pillar model.

This involves understanding the illness, peer support, connecting patients with their community, planning for future care, and developing an outcome-focused plan to support their hopes, desires, and aspirations.

You will be required to build and maintain relationships with colleagues within Alzheimer Scotland, the NHS, and other stakeholders, using your excellent communication skills and warm, flexible approach.

A full driving licence and access to a car are necessary for this post.

The successful candidate will be subject to a criminal check through Disclosure Scotland.

This is a permanent, part-time position (17.5 hours per week) with a salary range of £25,000 - £35,000 per year, depending on experience.

Applicants must hold a recognised relevant professional qualification, have a good understanding of dementia, and be able to evidence Enhanced Level skills in the Promoting Excellence Framework or be working towards its completion.
