We are seeking a skilled Post Diagnostic Support Link Worker to join our team in Edinburgh. As part of our National Service, you will work with individuals and their families to develop personalized support plans, enabling them to live well with dementia.
Key Responsibilities
- Develop and maintain relationships with colleagues, stakeholders, and service users
- Provide person-centered support to individuals with dementia and their families
- Work collaboratively with healthcare professionals to coordinate care and support
- Use digital platforms to facilitate virtual meetings and online groups
- Have a good understanding of dementia and its impact on individuals and families
- Hold a recognized professional qualification and have excellent communication skills
