Community Care Coordinator

We are looking for a Community Care Coordinator to join our team at Northwestern Community Services Board. As a key member of our team, you will work closely with families to provide support services and connect them with resources and opportunities.
Key Responsibilities- Develop and implement individualized care plans that meet the unique needs of each family.
- Provide emotional support and share knowledge with families to help them navigate complex service systems.
- Connect families with resources, opportunities, and other communities of support to enhance their well-being.
- A combination of training and experience indicating your ability to effectively perform essential job functions.
- A valid Driver's License and good driving record.
- The ability to provide own transportation.
Estimated salary: $45,000 - $55,000 per year, depending on qualifications and experience.
