Mental Health Care Coordinator

We are seeking a highly skilled Mental Health Nurse Practitioner to join our team at Northwestern Community Services Board, offering a competitive salary of $85,000 per annum.
The successful candidate will have graduation from an accredited nursing program and licensure as a Registered Nurse in Virginia, with experience in psychiatric nursing or recent EHR experience preferred.
- The chosen individual will provide direct and indirect care to agency consumers, prioritizing needs and treatment approaches to ensure appropriate delivery of care.
- This role involves coordination with physicians, nurse practitioners, and other agency and external service providers to ensure seamless care delivery.
- The selected candidate must possess a valid Driver's License, good driving record, and the ability to provide own transportation.
