About the Role

We are seeking an experienced and skilled Nurse Care Coordinator to join our team at Hampshire and Isle of Wight Healthcare NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in coordinating patient care and ensuring seamless transitions across healthcare settings.

Your primary responsibility will be to assess, plan, implement, and evaluate nursing care needs for patients on our 8-bed Mother and Baby unit. You will work closely with other healthcare professionals to develop and implement individualized care plans that promote optimal health outcomes.

In addition to your clinical expertise, you will have excellent communication and interpersonal skills, enabling you to effectively collaborate with colleagues, patients, and families. Your ability to mentor and supervise junior staff will also be essential in ensuring the success of our team.

Key Responsibilities
  1. To lead the clinical management of a designated group of service users in assessing, planning, implementing, and evaluating nursing care.
  2. To ensure that junior staff and students are actively engaged in patient activity.
  3. To observe, report, and record changes in the service user's physical, mental health, and behavior.
  4. To work with the MDT in planning and delivering care.
  5. To maintain working knowledge of legislation, policy, and guidance pertaining to mental health and learning disabilities.
Requirements
  • Registered Mental Health Nurse qualification.
  • Proven experience in coordinating patient care and working within a multidisciplinary team.
  • Excellent communication and interpersonal skills.
  • Able to work under pressure and prioritize tasks effectively.
