Neonatal Community Outreach Nurse

Barts Health NHS Trust is committed to providing high-quality patient care and is seeking a skilled Neonatal Community Outreach Sister to join our team. As a key member of our multidisciplinary team, you will play a vital role in delivering seamless, high-quality care to patients and their families.
Main Responsibilities- Provide expert care and support to patients and families, ensuring a smooth transition from hospital to home.
- Develop and implement community-based care plans, working closely with multidisciplinary teams to ensure comprehensive care.
- Teach and mentor nursing staff and families on matters relevant to discharge into the community, including BLS, NGT feeds, and self-confidence building.
- Liaise with multidisciplinary colleagues, including SLT and Dietician, to ensure comprehensive care.
- Participate in ward rounds and weekly MDT meetings to ensure effective communication and collaboration.
- Monitor the quality and standard of care provided by the outreach team, completing monthly audits to ensure excellence.

Person Specification- NMC registered
- Professional registration
- Teaching and mentoring qualification
- Specialist and intensive care experience
- BLS qualified
- NLS qualified
- Post-registration experience
- Experience at band 6
- Evidence of dissemination of knowledge to wider healthcare team
- Knowledge of implementation of community management plans
- Evidence of parent teaching and data collection database
- Sound clinical reasoning & decision making
- Ability to communicate clearly within the MDT colleagues and families
- Ability to work in a team
- Able to learn within a team and independently
- Able to escalate to appropriate manager any concerns
- Able to manage own time ensuring work done is to a high standard and in a timely fashion
- To be able to organise and prioritise own workload
