About the Role

We are seeking a highly skilled and experienced Neonatal Community Outreach Nurse to join our team at Barts Health NHS Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to neonates and their families in the community.

Key Responsibilities
  • Develop and implement community-based care plans for neonates and their families
  • Provide education and support to families on neonatal care and management
  • Collaborate with multidisciplinary teams to ensure seamless care transitions
  • Conduct regular audits to monitor the quality and standard of care provided
  • Participate in ward rounds and multidisciplinary team meetings
Requirements
  • NMC registration and professional registration
  • Teaching and mentoring qualification
  • Specialist and intensive care experience
  • Post-registration experience and experience at band 6
  • Ability to communicate clearly and work effectively in a team
