Neonatal Community Outreach Nurse

We are seeking a highly skilled and experienced Neonatal Community Outreach Nurse to join our team at Barts Health NHS Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to neonates and their families in the community.
Key Responsibilities- Develop and implement community-based care plans for neonates and their families
- Provide education and support to families on neonatal care and management
- Collaborate with multidisciplinary teams to ensure seamless care transitions
- Conduct regular audits to monitor the quality and standard of care provided
- Participate in ward rounds and multidisciplinary team meetings
- NMC registration and professional registration
- Teaching and mentoring qualification
- Specialist and intensive care experience
- Post-registration experience and experience at band 6
- Ability to communicate clearly and work effectively in a team
