Neonatal Critical Care Community Outreach Nurse Coordinator

We are seeking a highly skilled and experienced Neonatal Critical Care Community Outreach Nurse Coordinator to join our team. As a key member of our Neonatal Outreach team, you will play a vital role in supporting the care of critically ill neonates and their families.
Our Neonatal Service is the largest in North West London, providing care to high-risk, critically ill neonates. We are committed to integrating families into care, supporting parents in becoming partners in care through competency-based training, support, and education.
As a Neonatal Critical Care Community Outreach Nurse Coordinator, you will work across both hospital sites and in the community, developing your skills in assessing, planning, and facilitating discharges. You will be responsible for the assessment of care needs, the development, implementation, and evaluation of programmes of care without supervision in both the hospital and community setting.
Main Duties:- Assess care needs and develop programmes of care without supervision
- Coordinate the discharge of babies with ongoing special needs in the hospital and community setting
- Work in partnership with parents, empowering them to meet their baby's ongoing special needs following discharge
- Teach and supervise families, nursing and non-nursing staff, and be a resource person within the unit for discharge planning

Person Specification:- Mentorship in practice certificate ENB 998 or recognized Teaching and Assessing Certificate
- Qualification in community nursing
- Neonatal Community Module
- Valid UK driving license or equivalent
- Experience in managing a team
- Community experience
