Job Title: Neonatal Critical Care Community Outreach Nurse Co-Ordinator

We are seeking a highly skilled Neonatal Critical Care Community Outreach Nurse Co-Ordinator to join our team at Imperial College Healthcare NHS Trust. As a key member of our Neonatal Outreach team, you will play a vital role in assessing, planning, and facilitating discharges for babies with complex needs.

Main Responsibilities:
  • Assess care needs and develop, implement, and evaluate programmes of care without supervision in both hospital and community settings.
  • Work as part of a multi-professional team to coordinate the discharge of babies with ongoing special needs, both in the hospital and as part of the outreach service.
  • Teach and supervise families, nursing, and non-nursing staff, and be a resource person within the unit for discharge planning.
  • Deputise for the Lead Neonatal Critical Care Community Outreach Nurse as required.

Person Specification:
  • Essential:
    • RGN/RM/RSCN Neonatal qualification
    • Mentorship in practice certificate ENB 998 or recognised Teaching and Assessing Certificate
  • Desirable:
    • Qualification in community nursing
    • Neonatal Community Module
