About the Role
We are seeking a highly skilled and experienced Neonatal Community Outreach Sister to join our team at Barts Health NHS Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to newborn babies and their families in the community.
Key Responsibilities
- Develop and implement community management plans to ensure timely discharge of babies less than 36 weeks gestation.
- Provide training and support to families on matters relevant to discharge into the community, including BLS and NGT feeds.
- Work closely with multidisciplinary colleagues to ensure seamless care and support for families.
- Participate in ward rounds and weekly MDT meetings to ensure effective communication and collaboration.
- Monitor the quality and standard of care provided by the outreach team through regular audits.
