You will have the opportunity to work in both hospital and community settings, assisting in the assessment, planning, and facilitation of discharges. Main Responsibilities:
- Conduct comprehensive assessments of care needs
- Develop, implement, and evaluate care programs to ensure optimal outcomes
- Coordinate baby discharges with special needs, ensuring seamless transitions
- Provide education and supervision to families and staff, promoting continuity of care
- Support the development of community-based care initiatives
