Neonatal Critical Care Community Outreach Nurse Coordinator

We are seeking a highly skilled Neonatal Critical Care Community Outreach Nurse to join our team at Imperial College Healthcare NHS Trust.
As a key member of our Neonatal Outreach team, you will be responsible for assessing the care needs of critically ill neonates and developing programmes of care without supervision in both hospital and community settings.
You will work closely with parents and other healthcare professionals to ensure continuity of care and empower families to meet their baby's ongoing special needs following discharge.
The successful candidate will have a strong background in neonatal nursing, excellent communication skills, and the ability to work effectively in a team environment.

Requirements- RGN/RM/RSCN Neonatal qualification
- Mentorship in practice certificate ENB 998 or recognized Teaching and Assessing Certificate
- Valid UK driving license or equivalent
- Experience in managing a team and delivering teaching and training to all grades
