Neonatal Community Outreach Nurse

We are seeking a highly skilled and experienced Neonatal Community Outreach Sister to join our team at Barts Health NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality, patient-centered care to neonates and their families in the community.
Key Responsibilities- Develop and implement community-based care plans for neonates, ensuring seamless transitions from hospital to home.
- Provide education and support to families, empowering them to take an active role in their baby's care.
- Collaborate with multidisciplinary teams, including healthcare professionals, social workers, and community services, to ensure comprehensive care.
- Conduct regular home visits to assess the baby's progress, provide guidance, and address any concerns.
- Develop and maintain relationships with community-based services, ensuring a smooth referral process and optimal care for neonates.
- Participate in quality improvement initiatives, contributing to the development of best practices and policies.
- Registered Nurse with a specialization in Neonatal Care.
- Minimum 3 years of experience in neonatal care, with a focus on community-based care.
- Proven ability to work independently and as part of a team, with excellent communication and interpersonal skills.
- Strong knowledge of community-based care principles, including family-centered care and transitional care.
- Ability to work in a fast-paced environment, prioritizing tasks and managing time effectively.
