About the Role

We are seeking a highly skilled Neonatal Critical Care Community Outreach Nurse Coordinator to join our team at Imperial College Healthcare NHS Trust.

Main Responsibilities
  • Assess care needs and develop, implement, and evaluate programs of care without supervision in both hospital and community settings.
  • Work as part of a multi-professional team to coordinate the discharge of babies with ongoing special needs, both in the hospital and as part of the outreach service.
  • Teach and supervise families, nursing and non-nursing staff, and serve as a resource person within the unit for discharge planning.
Requirements
  • RGN/RM/RSCN Neonatal qualification
  • Mentorship in practice certificate ENB 998 or recognized Teaching and Assessing Certificate
  • Qualified in specialty > 3 years
  • Experience in managing a team
  • Holder of valid UK driving license or equivalent
  • Participate in clinical audit and clinical governance programs
  • Computer skills, experience of using databases
  • Experience with team working
  • Experience with delivering teaching and training all grades
