Neonatal Critical Care Community Outreach Nurse Coordinator

We are seeking a highly skilled Neonatal Critical Care Community Outreach Nurse Coordinator to join our team at Imperial College Healthcare NHS Trust.
Main Responsibilities- Assess care needs and develop, implement, and evaluate programs of care without supervision in both hospital and community settings.
- Work as part of a multi-professional team to coordinate the discharge of babies with ongoing special needs, both in the hospital and as part of the outreach service.
- Teach and supervise families, nursing and non-nursing staff, and serve as a resource person within the unit for discharge planning.
- RGN/RM/RSCN Neonatal qualification
- Mentorship in practice certificate ENB 998 or recognized Teaching and Assessing Certificate
- Qualified in specialty > 3 years
- Experience in managing a team
- Holder of valid UK driving license or equivalent
- Participate in clinical audit and clinical governance programs
- Computer skills, experience of using databases
- Experience with team working
- Experience with delivering teaching and training all grades
