Job Overview

Barts Health NHS Trust is seeking a highly skilled Neonatal Community Outreach Sister/Charge Nurse to join our team at the Royal London Hospital. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to neonates and their families in the community.

Main Responsibilities
  • Develop and implement community management plans to ensure seamless transitions from hospital to home care.
  • Provide expert teaching and support to families, nursing staff, and multidisciplinary colleagues on matters relevant to discharge into the community.
  • Liaise with multidisciplinary colleagues to ensure comprehensive care and support for neonates and their families.
  • Participate in ward rounds and weekly multidisciplinary team meetings to ensure effective communication and collaboration.
  • Monitor the quality and standard of care provided by the outreach team through regular audits and quality improvement initiatives.
Requirements
  • NMC registered and professional registration.
  • Teaching and mentoring qualification.
  • Significant post-registration experience, preferably at Band 6.
  • Experience in community follow-up/visits and parent BLS teaching.
  • Sound clinical reasoning and decision-making skills.
