Imperial College Healthcare NHS Trust: Neonatal Outreach Nurse Opportunity
We are seeking a skilled and qualified neonatal nurse to join our team in providing high-quality care to high-risk neonates in North West London. As a Neonatal Outreach Nurse, you will have the opportunity to work in both hospital and community settings, assessing care needs, developing and implementing care programs, and coordinating baby discharges with special needs.
Main Responsibilities:
- Conduct thorough assessments of care needs to ensure optimal patient outcomes
- Develop, implement, and evaluate comprehensive care programs to meet the unique needs of each patient
- Coordinate with multidisciplinary teams to facilitate smooth baby discharges with special needs
- Provide education and supervision to families and staff to promote continuity of care
- Support the development of a family-centered approach to care, empowering parents to be actively involved in their baby's care
Requirements:
- Qualified neonatal nurse with over 3 years of experience
- Strong leadership skills and ability to work collaboratively with multidisciplinary teams
- Excellent communication and interpersonal skills
