Job Overview:
Care Homes and the Community are seeking a highly skilled Advanced Nurse Practitioner to join their team. As an experienced professional, you will have the opportunity to work in a purpose-designed residential setting, delivering person-centered dementia care and therapeutic interventions.

About the Role:
This is a challenging yet rewarding role, where you will support and coordinate care for individuals living with complex dementia presentations. You will work closely with a multidisciplinary team, providing general nursing and bio-psychosocial interventions, and using critical reflective practice to inform your decision-making.

Essential Skills and Qualifications:
  • Evidence of delivering person-centered care and care for people with dementia who present with complex needs, distress, and behaviors that challenge.
  • Evidence of sharing knowledge and delivering best practice.
  • Evidence of using critical reflective practice.
  • Collaborative and MDT working.
  • Evidence of continued professional development.
  • Experience of providing general nursing and bio-psychosocial interventions.
  • Experience of working as an autonomous practitioner.

Desirable Skills and Qualifications:
  • Knowledge of relevant current dementia policies and guidance.

Salary: £40,000 - £55,000 per annum.
