About the Role

We are seeking a skilled and compassionate Dementia Nurse Practitioner to join our dedicated team in a newly opened Dementia care community within our home.

Key Responsibilities
  • Conduct comprehensive assessments and develop personalized care plans for individuals with dementia.
  • Deliver evidence-based interventions to improve the well-being and quality of life of our residents.
  • Provide guidance, education, and mentoring to care staff to ensure the highest standards of care.
  • Collaborate with health, social, and voluntary services to ensure coordinated care.
Requirements
  • Registered Nurse (RMN/RGN/RNLD) qualified.
  • Minimum 2 years of experience in dementia care.
  • Strong communication and interpersonal skills, with a compassionate approach.
  • Full eligibility to work and live in the UK.
