About the Role

We are seeking a dedicated and compassionate Support Worker Team Leader to join our team in supporting a client in their own home. This role requires a Personal Assistant to provide high-quality care and support to our client, helping them to maintain their independence and quality of life.

The successful candidate will be responsible for providing Support and Care to our client, including assistance with daily living tasks, managing their medication, and facilitating their participation in social and leisure activities. As a Team Leader, you will also be responsible for supervising and mentoring a team of support workers, ensuring that they are equipped to provide the highest standard of care.

We are looking for a candidate with excellent Communication Skills and a strong understanding of the needs of people with disabilities. The ideal candidate will be a team player, able to work effectively in a fast-paced environment and provide a high level of care and support to our client.

Key Responsibilities:

  • Providing Personal Care and support to our client, including assistance with daily living tasks, managing their medication, and facilitating their participation in social and leisure activities.
  • Supervising and mentoring a team of support workers, ensuring that they are equipped to provide the highest standard of care.
  • Developing and implementing Care Plans that meet the individual needs of our client.
  • Monitoring and evaluating the effectiveness of care and support provided, making recommendations for improvement as necessary.
  • Working closely with other healthcare professionals to ensure that our client receives a comprehensive and coordinated package of care.

Requirements:

  • A minimum of 12 months experience in a care or support role.
  • Excellent Communication Skills and a strong understanding of the needs of people with disabilities.
  • Ability to work effectively in a fast-paced environment and provide a high level of care and support to our client.
  • Knowledge of Care Standards and regulations, including the Care Quality Commission (CQC) standards.
  • Ability to work collaboratively as part of a team and provide supervision and mentoring to support workers.
