About the Role:

Perthyn is seeking a highly skilled and dedicated Supported Living Manager to join our team in Powys. As a key member of our management team, you will be responsible for leading a team of support workers and ensuring that individuals with complex needs receive high-quality care and support.

Key Responsibilities:

  • Lead a team of support workers to deliver person-centred care and support plans
  • Work closely with the Regional Manager to ensure consistency in the implementation of support plans
  • Develop and maintain positive relationships with families, other agencies, and services
  • Ensure that staff have the necessary skills and resources to deliver high-quality care and support
  • Monitor and evaluate the effectiveness of support plans and make recommendations for improvement

Requirements:

  • Minimum QCF Level 3 or equivalent in Health and Social Care
  • Experience of working with individuals with complex needs and learning disabilities
  • Excellent leadership and communication skills
  • Ability to work positively with other agencies and services
  • Reliability and flexibility to meet the needs of the role
