About the Role

We are seeking a skilled and compassionate Health and Well-being Worker to join our Forensic Community Service team at Lancashire & South Cumbria NHS Foundation Trust.

Job Summary

The successful candidate will work independently with service users in a variety of settings, under the guidance of the Multi-Disciplinary Team (MDT). They will meet regularly with the supervising clinician to review work and highlight individual's changing needs or wider service issues affecting an individual's care.

Main Responsibilities
  • Support individuals to achieve optimum potential by promoting independence and self-care as appropriate.
  • Support social inclusion and act as an advocate to facilitate views and choices expressed by individuals.
  • Develop rapport with individuals and encourage their interest and participation in specified therapeutic activities.
  • Facilitate group and individual interventions as directed by the MDT to support service user recovery plans.
About the Team

The Forensic Community Service is expanding, and we are excited to introduce a brand-new role that will support our commitment to holistic care. Our team is dedicated to providing high-quality care to adults with mental disorders who have transitioned from secure mental health settings into the community.

Requirements
  • NVQ Level 2 or higher in Health and Social Care or equivalent.
  • Ability to undertake Violence Reduction physical intervention training.
  • Good communication, self-management, and networking skills.
