Disability Services Coordinator

Role Summary:
Acuity Care Group Limited is seeking a highly motivated and experienced Disability Services Coordinator to join our team. As a key member of our management team, you will be responsible for coordinating the delivery of high-quality care and support services to individuals with complex needs.
Key Responsibilities:
- COORDINATE THE DELIVERY OF CARE AND SUPPORT SERVICES TO INDIVIDUALS WITH COMPLEX NEEDS.
- DEVELOP AND IMPLEMENT CARE PLANS IN COLLABORATION WITH INDIVIDUALS, THEIR FAMILIES, AND MULTIDISCIPLINARY TEAMS.
- Maintain accurate records and reports, ensuring compliance with regulatory requirements.
- Collaborate with other departments to ensure seamless service delivery.
- Promote a culture of empowerment, choice, and inclusion among the individuals we support.
Requirements:
- Degree-level qualification in a relevant field (e.g., nursing, social work, psychology).
- At least 2 years' experience in a leadership role within the healthcare sector.
- Proven track record of successfully managing a team and improving outcomes.
- Excellent communication, interpersonal, and problem-solving skills.
- Able to work independently and as part of a team.
