Care Team Coordinator

We are seeking an experienced Team Leader to join our care team in Exeter. As a key member of our team, you will be responsible for supporting an individual with Autism and complex care needs.
The ideal candidate will have experience working in the healthcare sector, specifically with individuals with Autism, Learning Disabilities, and Mental Health conditions.
Key responsibilities include:
- Supporting the individual with daily living tasks and promoting their independence
- Fostering a supportive and inclusive environment
- Ensuring the individual's safety and well-being at all times
- Communicating effectively with the individual and other team members
Required Skills and Qualifications
You will need:
- Team leader experience within the health care sector
- Autism experience
- Learning disability experience
- Mental health experience
- Able to effectively communicate with individuals requiring care
