Job Overview:
We are seeking an experienced Care Team Supervisor to join our team at Acuity Care Group Limited. As a key member of our management team, you will play a vital role in delivering high-quality care and support to individuals with complex needs.
Key Responsibilities:
- SUPERVISE AND MANAGE A TEAM OF SUPPORT WORKERS TO ENSURE THE DELIVERY OF HIGH-QUALITY CARE AND SUPPORT.
- DEVELOP AND IMPLEMENT CARE PLANS IN COLLABORATION WITH INDIVIDUALS, THEIR FAMILIES, AND MULTIDISCIPLINARY TEAMS.
- Maintain accurate records and reports, ensuring compliance with regulatory requirements.
- Collaborate with other departments to ensure seamless service delivery.
- Promote a culture of empowerment, choice, and inclusion among the individuals we support.
Requirements:
- Degree-level qualification in a relevant field (e.g., nursing, social work, psychology).
- At least 2 years' experience in a leadership role within the healthcare sector.
- Proven track record of successfully managing a team and improving outcomes.
- Excellent communication, interpersonal, and problem-solving skills.
- Able to work independently and as part of a team.
