Job Title: Care Manager

We are seeking a skilled Care Manager to join our team at Acuity Care Group Limited.

The ideal candidate will have experience in the healthcare sector, specifically as a Team Leader.

This role involves supporting an individual living in Fordingbridge with Autism, who is active and loves going out with their dog and into the community.

As a Care Manager, you will be responsible for providing companionship, support with daily living tasks, and promoting empowerment through tailored care plans.

You will work closely with the individual and other team members to ensure a supportive and inclusive environment.

Your key responsibilities include:

  • Providing emotional support and companionship to the individual.
  • Supporting with personal care and housekeeping tasks.
  • Fostering a supportive and inclusive environment.
  • Ensuring the individual's safety and well-being at all times.
  • Communicating effectively with the individual and other team members.
  • Maintaining confidentiality and respecting privacy.

The successful candidate will have:

  • Experience as a Team Leader in the healthcare sector.
  • Autism experience.
  • Learning disability experience.
  • Mental health experience.
  • Strong attention to detail and adherence to safety protocols.
  • Empathy, patience, and compassion when interacting with individuals in need of support.
