About the Role

We are seeking a highly skilled and dedicated Social Worker to join our Dewsbury Community Wellbeing Team. As a key member of our team, you will be responsible for undertaking initial assessments, reviews, and safeguarding enquiries for vulnerable adults, working closely with their informal and formal carers in the community.

Key Responsibilities
  • Undertake initial assessments, reviews, and safeguarding enquiries for vulnerable adults
  • Work with adults over the age of 18 with physical, sensory disabilities, and mental health needs
  • Engage in multi-agency working with a range of allied health and social care professionals to achieve the best outcomes for people
  • Make eligibility determinations as defined in the Care Act (2014) and safeguarding
  • Use outcome-focused assessment and knowledge of the Mental Capacity Act (2005) to adopt a positive approach to risk
Requirements
  • Level 2 or Level 3 Social Worker qualification
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively
  • Knowledge of the Care Act (2014) and Mental Capacity Act (2005)
  • Experience of working with adults with physical, sensory disabilities, and mental health needs
