Job Title: Trusted Assessor

Kirklees Council is seeking a skilled and experienced Trusted Assessor to join our team. As a Trusted Assessor, you will play a critical role in supporting the discharge process for patients in hospitals across Kirklees, Calderdale, and Wakefield.

Job Summary:

We are looking for a highly motivated and organized individual to work collaboratively with partners across the social care and health system to deliver effective timely discharges to Care Homes. The ideal candidate will have relevant experience and understanding of the issues faced by the care sector, with applications especially welcome from individuals with nursing, care home management, or social work experience.

Key Responsibilities:
  • Collaborate with partners across the social care and health system to deliver effective timely discharges to Care Homes.
  • Work with care home managers to gather information for assessments and support the discharge process.
  • Develop and maintain relationships with care home providers and other stakeholders to ensure smooth discharge processes.
  • Contribute to the development and implementation of strategies to improve discharge processes and outcomes.
Requirements:
  • Relevant experience and understanding of the issues faced by the care sector.
  • Strong interpersonal and organizational skills.
  • Ability to work collaboratively as part of a broader team.
  • Excellent communication and problem-solving skills.
