About the Job:

This is an excellent opportunity for a caring and compassionate individual to undertake strength-based, outcome-focused assessments and support planning, ensuring that residents receive the right amount of support.

You will play a vital role in supporting people to access local services, maintain independence, promote health and wellbeing, and to live independently.

Key Responsibilities:

  • To conduct comprehensive assessments of individuals' needs and develop personalized support plans
  • To work collaboratively with other agencies and stakeholders to ensure seamless service delivery
  • To provide guidance and support to individuals and their families

Salary and Benefits:

The annual salary for this position is £35,235 - £39,513, plus a range of benefits including a generous Local Government Pension Scheme with employer contribution, relocation reimbursement package, and access to dozens of discounts through Vivup, our employee benefits partner.
