Are you a motivated social worker looking to make a difference in the lives of individuals in East Lancashire?
We are seeking a permanent, full-time Adult Social Worker to join our team, working across East Lancashire as part of our Adult Services Older Adults and Physical Disability Community Teams.
About the Role:
This is an exciting opportunity for a skilled social worker to undertake strength-based, outcome-focused assessments and support planning, ensuring that residents receive the right amount of support.
You will play a vital role in supporting people to access local services, maintain independence, promote health and wellbeing, and to live independently.
In return for your commitment and hard work, you will work alongside supportive managers and be offered various training and progression opportunities.
Key Responsibilities:
- To conduct comprehensive assessments of individuals' needs and develop personalized support plans
- To work collaboratively with other agencies and stakeholders to ensure seamless service delivery
- To provide guidance and support to individuals and their families
Requirements:
You must be registered with Social Work England (SWE) to be appointed as a Social Worker or have applied to register with SWE. A car for business use is also required, but we may consider candidates who cannot drive due to disability.
