About the Role

This is an exciting opportunity to join our team as a Social Care Worker - Community Based, delivering person-centered support services to vulnerable individuals in our community-based settings.

As a key member of our team, you will work collaboratively with colleagues and external partners to develop and implement effective support plans, providing emotional support, guidance, and practical assistance to help individuals overcome challenges and achieve their goals.

The successful candidate will possess excellent communication skills, empathy, and a strong understanding of the challenges faced by vulnerable communities. You will work flexibly, adapting to changing circumstances and priorities, while maintaining accurate records and reports, ensuring timely submission and adherence to regulatory requirements.

Key Responsibilities:

  • Deliver person-centered support services to vulnerable individuals, focusing on empowerment, independence, and quality of life.
  • Collaborate with colleagues, families, and external agencies to develop and implement effective support plans.
  • Provide emotional support, guidance, and practical assistance to help individuals overcome challenges and achieve their goals.
  • Maintain accurate records and reports, ensuring timely submission and adherence to regulatory requirements.
  • Participate in training and development opportunities to enhance your skills and knowledge, staying up-to-date with best practices and industry developments.
