Job Description: Disability Support Lead

We are seeking a dedicated and compassionate Disability Support Lead to join our team at Midland Mencap in Birmingham. As a Disability Support Lead, you will be responsible for providing high-quality support to adults with learning disabilities, enabling them to achieve their goals and improve their quality of life.

Main Responsibilities:
* Deliver person-centred support that meets individual needs and risk assessments
* Develop and maintain relationships with service users, their families, and carers
* Work collaboratively with colleagues to ensure seamless support delivery
* Identify opportunities for individuals to develop new skills and interests
* Monitor progress, set goals, and evaluate success
* Maintain accurate records and adhere to organisational policies
