We are seeking a highly skilled and experienced Nurse Assessor to join our team at NHS Coventry & Warwickshire Integrated Care Board. As a key member of our Learning Disability Team, you will play a vital role in ensuring the quality of care and contributing to the culture of collaboration, creativity, and hard work.
Key Responsibilities- Facilitate multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures.
- Support the Complex Case Manager/Clinical Commissioning Manager and the wider team to ensure that the workstream is planned and managed effectively and ensure successful delivery.
- Manage cases as directed by the Management Team, including resolving issues with funding, placements, invoices, and complaints.
- Ensure Continuing Healthcare guidance is followed and that services are delivered to maximise patient safety.
- Support the development of personalisation across the organisation.
- Provide accurate and legible recording of information.
- Assist/support individuals and representatives in relation to the NHS Continuing Healthcare process.
- Operate within and provide enhancements to current management information, reporting to enhance decision making processes.
- Contribute to the development of performance and governance strategies and the development and implementation of improvement programmes.
- Deliver against objectives, performance targets, achieving quality outcomes, prioritising own workload, and working to tight deadlines.
- Support training and induction of new staff.
- Undertake research and development as directed.
- Registered Learning Disability Nurse or other part of register with significant LD experience.
- Evidence of post-qualifying and continuing professional development.
- Clear communicator with excellent writing and report writing skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences.
- Ability to work on own initiative and organise own workload without supervision working to tight and often changing timescales.
- Car driver/owner or reasonable alternative.
- Able to demonstrate understanding of other relevant legislation, MHA.
- Previous experience in similar role in public sector.
- Able to demonstrate understanding of MCA, DOLS, and CHC Framework.
