We are recruiting a highly motivated Youth Care Coordinator to join our team at Polaris Community in London. In this role, you will provide vital support services to young people in their supported living homes and the community, focusing on their safety and well-being.

Key aspects of this position include:

  • Delivering a high-quality, responsive, and customized support service, tailored to individual needs
  • Maintaining strong relationships with young people, staff, and external agencies, ensuring seamless communication
  • Adhering to person-centred risk assessments and support plans to ensure continuous support

Successful candidates will have experience supporting vulnerable young people, be flexible with their schedule, including weekends, and hold relevant qualifications, such as a Residential Childcare/Families Practitioner Diploma or equivalent. A full driving license and access to a vehicle are also essential.

We prioritize the safety and well-being of children, young people, and vulnerable adults, expecting all staff to adhere to these values. The successful candidate will undergo an enhanced DBS check for this role.

Salary: £12.02 per hour
