This is an exciting opportunity to join Hatched Recruitment Group as a youth care transition coordinator. In this role, you will work closely with young people in care and leaving care, helping them navigate the transition from care to independent living.

Responsibilities
  • Work with young people in care and leaving care to develop, monitor, and review pathway plans of support and interventions.
  • Collaborate with local authorities, social workers, and other stakeholders to ensure a smooth transition process.
  • Provide ongoing support and guidance to young people during their transition period.
Requirements
  • Degree in Social Work or related field.
  • Experience working with young people in care and leaving care.
  • Knowledge of child care legislation, including the Children Act 1989 and associated guidelines and regulations.
Salary and Benefits

£38,000 - £42,000 per annum, depending on experience.
