Job Description: Community Healthcare Coordinator

We are looking for a skilled Pediatric Nurse to join our team as a Community Healthcare Coordinator. The successful candidate will be responsible for providing face-to-face support to children and their families, creating personalized care plans, and training nursing staff.

The ideal candidate should have a RSCN qualification, an active NMC PIN, and up-to-date experience of tracheostomy care and ventilators within the last 2 years. They should also have effective leadership and management skills and be able to work closely with community teams, CHC nurses, and CCG commissioners.

This full-time role offers a competitive salary of £30,000 - £42,000 per annum, depending on experience, plus benefits including 33 days holiday, a private pension scheme, and use of a company car or travel card for community travel. The working hours are Monday to Friday (9-5) with opportunities for flexible working arrangements.
