We are looking for a Children's Healthcare Professional to join our team as a Case Load Coordinator. This is an exciting opportunity to work with children and their families in the North East London area.
About the Job:
- Responsibilities include designing and delivering care plans, providing ongoing monitoring and support, and delivering nursing interventions
- Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain packages of care
Key Skills and Qualifications:
- RSCN with a minimum of 6 months' experience in the UK and an active NMC PIN
- Experience of leading HCAs and supporting management style
- Up-to-date experience of tracheostomy care and ventilators within the last 2 years
Benefits and Salary:
- Estimated salary: £42,000 - £50,000 per annum (dependent on experience)
- 2 days working from home per week / 3 days in the community
- Monday to Friday (9-5) hours
- 33 days holiday
- Private pension scheme
- Use of pool company car or travel card for community travel
- Business travel expenses
