Family Wellbeing Coordinator Job Description

Position Overview

We are seeking a highly skilled and compassionate Family Wellbeing Coordinator to promote health and wellbeing within the community, particularly for families with young children.

Responsibilities:

  1. Conduct home visits to assess the health and developmental needs of children and families.
  2. Develop and implement individual care plans tailored to meet the specific needs of families.
  3. Provide education on health promotion, parenting skills, and child development.
  4. Collaborate with other healthcare professionals to ensure comprehensive care for families.
  5. Lead community health initiatives to raise awareness about available services and resources.
  6. Support families in accessing additional services as needed, including mental health support and social services.
