About the Role

The Pediatric Care Specialist plays a vital role in promoting health and wellbeing within the community, particularly for families with young children. This position involves conducting home visits to assess the health and developmental needs of children and families, developing individual care plans tailored to meet their specific needs, and providing education on health promotion, parenting skills, and child development.

Duties and Responsibilities

  • Conduct home visits to assess the health and developmental needs of children and families.
  • Develop and implement individual care plans tailored to meet the specific needs of families.
  • Provide education on health promotion, parenting skills, and child development.
  • Collaborate with other healthcare professionals to ensure comprehensive care for families.
  • Lead community health initiatives to raise awareness about available services and resources.
  • Support families in accessing additional services as needed, including mental health support and social services.

Requirements

  • Registered Nurse qualification with experience in community health or paediatrics preferred.
  • Strong leadership skills to guide families through health challenges.
  • Familiarity with developing care plans that address individual family needs.
  • A compassionate approach with a commitment to promoting the wellbeing of children and families.
