Mental Health Specialist Care Coordinator

We are seeking an experienced Mental Health Specialist Care Coordinator to join our team at NELFT North East London Foundation Trust. As a key member of our Early Intervention in Psychosis (EIP) service, you will play a vital role in providing high-quality care and support to individuals experiencing psychosis.
About the RoleThis is a challenging yet rewarding opportunity for a skilled professional to make a meaningful difference in the lives of our service users. As a Mental Health Specialist Care Coordinator, you will be responsible for coordinating care and providing support to individuals with complex mental health needs.
Key Responsibilities- Coordinate care planning and implementation for service users, ensuring seamless transitions between services
- Work collaboratively with multidisciplinary teams to provide comprehensive support and guidance to service users and their families
- Develop and maintain effective relationships with service users, carers, and other stakeholders to ensure holistic care
- Conduct risk assessments and develop safety plans to mitigate potential risks and promote well-being
To succeed in this role, you will need:
- Degree in a relevant field (e.g., nursing, psychology, social work)
- Post-qualification experience in mental health care, preferably in EIP or similar settings
- Strong knowledge of government legislation and policy guidelines related to mental health and EIP
- Excellent communication and interpersonal skills, with the ability to build strong relationships with diverse stakeholders
- Clinical risk assessment and management skills, with experience in developing and implementing safety plans
NELFT offers a competitive salary and benefits package, including:
- An estimated salary of £45,051 - £51,039 per annum, depending on experience
- A High Cost Area Supplement of 15% of basic salary (minimum payment of £4,551 and maximum payment of £5,735 per annum pro rata)
