Disability Care Coordination Professional

A highly skilled Social Worker is required to join our team in the Children & Young People with Disability Service.
This is a unique opportunity to drive positive change in the lives of our service users and their families.
You will be responsible for reviewing care packages, developing innovative support plans, and ensuring the child's voice is paramount in all decision-making.
Main Responsibilities:
- Reviewing care packages and designing individualised support plans
- Building strong relationships with colleagues and partners to deliver high-quality services
- Fostering collaboration between internal and external stakeholders to improve service effectiveness
- Creating comprehensive reports for senior managers
Our salary range is £39,951 - £46,041, accompanied by a £2,500 retention allowance, a generous pension, relocation package, and 27 days' annual leave, as well as opportunities for professional development.
