About the Role:

  1. You will work closely with reablement providers and the voluntary sector to support individuals regain independence through a strength-based approach.
  2. Assist with goal planning for short-term care after hospital discharge or to prevent hospital admission.
  3. Use your expertise in activity analysis, assistive technology, and minor adaptations to reduce formal care.
  4. Apply moving and handling skills as part of your role.
