Patient Care Coordinator

Key Responsibilities:
• Coordinate patient transitions between pediatric and adult services, ensuring seamless care continuity.
• Develop and implement personalized care plans, focusing on patient-centered care and optimal outcomes.
• Collaborate with multidisciplinary teams to ensure effective communication and care coordination.

Requirements:
• Strong clinical knowledge and experience in IBD care
• Excellent communication and organizational skills
• Ability to work effectively in a team environment

Salary: £38,000 - £48,000 per annum, depending on experience.
