The post of Patient Care Coordinator plays a vital role in facilitating timely and effective discharge of women and babies from the hospital. As a key member of our team, you will be responsible for assessing and planning for patient discharge, taking into account individual needs and circumstances.

This is a 30 hour per week contract, offering a competitive salary of £26,530 - £29,114 per annum pro rata. We are seeking an experienced professional with excellent communication skills and a strong ability to work collaboratively with healthcare staff.

Your responsibilities will include:

  • Assessing patient needs and developing personalized discharge plans.
  • Collaborating with healthcare staff to ensure smooth transitions between care settings.
  • Maintaining accurate records and reports to inform discharge decisions.
