Band 5 Endoscopy Care Coordinator

We are seeking a highly motivated and organized individual to fill the role of Band 5 Endoscopy Care Coordinator. The successful candidate will have a passion for delivering high-quality patient care, excellent communication skills, and the ability to work effectively in a fast-paced environment.
Your primary responsibilities will include:
- Coordinating patient care throughout the pre-assessment pathway, including assessment, preparation, and post-procedure follow-up.
- Providing expert nursing advice, education, and support to patients and healthcare staff, promoting evidence-based practice and contributing to the development of clinical guidelines.
- Collaborating with multidisciplinary teams to ensure seamless transitions and optimal outcomes, including communication with patients, families, and healthcare professionals.
- Participating in quality improvement initiatives, identifying areas for improvement, and implementing changes to enhance patient care and service delivery.
- Maintaining accurate records, reports, and other documents as required, adhering to trust policies and procedures.
You will be expected to work collaboratively with colleagues from various disciplines, demonstrating excellent communication, interpersonal, and problem-solving skills.
